60. The Fill Your Clinic Bootcamp: The Marketing Playbook You Wish You Had From Day One

You spent over a decade training to be an excellent clinician. You know how to diagnose, treat, and genuinely care for your patients. So why is your practice’s clinical schedule half empty?

If you've started your own practice and the patients aren't showing up the way you expected, you've probably run through the mental checklist a hundred times. 

Should I raise my prices? Hire a marketing company? Finally get serious about social media? Is something wrong with me?

No, nothing is wrong with you. You were simply never taught how to grow a business. Medical training teaches you everything about patient care and nothing about patient acquisition.

And what makes it worse? Back when you worked in a corporate system, someone else was doing the marketing for you. Airport ads, brand campaigns, referral networks, millions in spend you never saw. Patients didn't just appear because you were good. They appeared because a machine was running behind the scenes.

Now that machine is gone, and it's just you.

In this post, I'm breaking down the biggest reasons talented clinicians struggle to fill their practices, the mindset traps that keep them stuck, and what actually works instead.

The marketing machine you never saw

When you worked in corporate medicine, you only saw the execution side. What you didn't see was the enormous operation generating your patient flow.

Big hospital systems spend millions on brand campaigns. They buy up primary care clinics, not because they're profitable, but because they feed the referral chain: primary care refers to a specialist in the system, the specialist refers to imaging at the hospital, imaging leads to a surgeon down the hall. It's a self-referring ecosystem, and when you worked inside it, a huge portion of your patients arrived through that pipeline without you lifting a finger.

They also had something you have to build from scratch: borrowed trust. Put the word "University" in front of a clinic name and patients assume higher-level care, even when the clinicians are exactly the same.

So if patients aren't flooding in now, it's not because you got worse at medicine. It's because the machine is gone.

It takes far more visibility than you think

When I was in business school, the standard was that someone needed to see a brand seven times before making a purchase. Today's estimates? Over to twenty.

That number tends to shock clinicians, and it explains a common trap: you say something once, feel like everyone must be sick of hearing it, and stop. Meanwhile, on the receiving end, almost nobody registered it at all. People are flooded with information all day. The first several touchpoints barely land.

The fix isn't shouting louder, but showing up consistently and cohesively, with a clear message, more times than feels comfortable.

The mindset traps keeping you stuck

Beyond the structural stuff, there are a few mental patterns we see over and over:

  • "Marketing is yucky." Many clinicians leave training with an us-versus-them view of business. But marketing is not sales. Marketing is building awareness, letting people know you exist as an option, sometimes making them aware they even have a problem worth solving.
  • Staying silent. Silence doesn't mean people have no thoughts about you. If you don't own your narrative, patients fill in the blanks, and they'll assume you're just another rushed, insurance-based clinic, even if you offer longer visits and deeper care.
  • Over-indexing on efficiency. Corporate medicine put you on a treadmill where anything slow got cut or outsourced. But year one of a business is inherently inefficient. You're learning a new language, and the early vocabulary is tedious. Yes, the early reps feel basic and slow. But that means you're actually learning instead of outsourcing.

Why throwing money at it backfires

The most expensive mistake we see is outsourcing marketing too early. Clinicians hand everything to an agency on a $5,000 monthly retainer, and months later they've spent $25,000 with little to show for it.

The real cost isn't just the money. When you outsource the task, you outsource the learning. You walk away still not knowing your key messages, your audience, or how to talk to referral partners. And if that agency is overseas because it's cheaper, they won't understand the nuances of US healthcare, which, frankly, doesn't make sense to anyone.

For at least your first year, stay in control of your own marketing. Nobody will ever care about your clinic as much as you do.

What actually works

The good news: this is learnable, and there's a repeatable playbook.

  • Know your patient pipelines. There are seven distinct pipelines where patients come from. Some clinicians build thriving practices on personal networks, others on social media. You don't need all seven. You need to experiment and find yours.
  • Use the scientific method. You spent years learning it, and it works in business too. Test, measure, keep what works, cut what doesn't.
  • Track your energy, not just your metrics. Marketing you dread won't be sustainable. Marketing that energizes you will. Pay attention to both.
  • Get clear on your story. Why would someone choose you over a generic insurance-based clinic? You need to be so clear on the answer that you've sold yourself first.

And one more thing: do it alongside other people. Learning with a community of clinicians who also feel awkward and nervous about marketing accelerates everything. The results, the experience, and the growth are all better together.
